What Is a Loss Ratio?

Let’s say you get ten dollars a week for snacks. If you spend eight dollars on chips and candy and save the other two, you know how much of your money went toward snacks.

That is kind of what a Loss Ratio tells you, how much money was spent on health care compared to how much was collected.

So what is it really?

In health insurance, a Loss Ratio is the percentage of premium dollars that are used to pay for medical claims. If a company pays one hundred thousand dollars in premiums and the insurance company spends eighty thousand dollars on employee medical bills, the loss ratio is eighty percent.

It shows how much of the money collected is being used for care versus how much is going toward things like administrative costs, profit, or savings.

Why does it matter?

Loss Ratio helps employers and advisors understand how well a health plan is performing. A very high ratio could mean the plan is paying out a lot in claims and costs might go up. A very low ratio might suggest that employees are not using their benefits much or that the plan is priced too high.

In some cases, a certain loss ratio must be met to qualify for refunds or rebates. This is especially true in traditional fully insured plans, where insurance companies are required to spend a certain amount of premium dollars on care.

How is it used?

For self-funded or level-funded plans, the loss ratio helps guide decisions. Employers may look at it to see if they are getting good value, if they should adjust plan design, or if wellness efforts are working. It also plays a role in renewal discussions and stop-loss strategies.

Simply put:

Loss Ratio shows how much of your health care dollars are being used to pay for care. It is a helpful way to measure plan performance and make better decisions for the future.
